Forms we need to have on file by the opening of school:

(Each of the following pdf files can be viewed with Adobe Acrobat Reader, downloaded and printed)
  1. Child Enrollment and Health Information (JFS01234) – new every new school year.
  2. Child Medical Statement (JFS01305 ) which is valid for 13 months from the last well child examination.
  3. Medical/Physical Care Plan (JFS01236) if your child has a special need, and if needed, Request for Administration of Medication (JFS01217).
  4. Also, we require parents of newly enrolled children to fill out the Personal Questionnaire.
  5. Please fill out a media release form.

Authorization Forms for Tuition

  1. Recurring Credit Card Authorization Form for Tuition.
  2. Recurring Checking Account Debit Form for Tuition.
