Forms we need to have on file by the opening of school:
(Each of the following pdf files can be viewed with Adobe Acrobat Reader, downloaded and printed)
- Child Enrollment and Health Information (JFS01234) – new every new school year.
- Child Medical Statement (JFS01305 ) which is valid for 13 months from the last well child examination.
- Medical/Physical Care Plan (JFS01236) if your child has a special need.
- Also, we require parents of newly enrolled children to fill out the Personal Questionnaire.
- Please fill out a media release form.
