Forms Needed Prior to Attending Preschool
Child Enrollment and Health Information (JFS01234) – updated every new school year.
- Child Medical Statement (JFS01305) which is valid for 13 months from the last well child examination.
- Medical/Physical Care Plan (JFS01236) if your child has a special need, and Request for Administration of Medication (JFS01217) if needed.
- Also, we require parents of newly enrolled children to fill out the Personal Questionnaire.
- Please fill out a media release form and a Pick-up List form.
